I authorise the teachers to obtain medical assistance where they deem necessary should an accident occur. I agree to pay all medical expenses incurred on behalf of the above student and carer. I submit the above medical information about my child and include other relevant information and details of limitations which he/ she has for the activity concerned. I further authorise qualified practitioners to administer anaesthetic if such an eventuality arises. I agree that I will be there during the whole lesson and take full responsibility for any injury that may occur and understand that INVY SWIMMERS, its servants, directors, employees and agents are not liable for any injuries that may occur during BEFORE AND AFTER child’s lesson time.

I understand that the information I have provided is collected and held in accordance with the provisions of the privacy act. I understand the information that I have provided is necessary for the service to be provided. I acknowledge and agree that the information will only be used to facilitate the service and to advise me of matters relating to the service, I understand that I will be able to access and alter information that has been supplied.

I understand that term fees are payable upfront and that my child/children will roll over into their positions from term to term. I understand that as an existing customer payment must be made one week prior to the commencement of the next term* to secure enrolment for the following term. I understand that as a new customer fees must be paid prior to participation of the first class. I understand that failure to do so will result in my child/children’s position/s being open for new enrolments and my child/children will be placed on the wait list.

I understand that no refund or credit will be given for withdrawal due to change of mind. I understand that in the case of illness or extenuating circumstances a partial credit may be given, with the deduction of a cancellation fee of $50. I understand that I may need to provide evidence of my circumstances e.g. medical certificate.

I understand that my enrolled child/children are permitted to swim in the space provided before and after their lessons, that their safety and conduct are the responsibility of myself (or their carer) and agree to supervise them at all times.

I understand that there is no lifeguard on premises.
